Privacy & HIPAA Notice

This notice explains how Moll Premier Primary Care collects information from this website and how our office handles protected health information under the Health Insurance Portability and Accountability Act (HIPAA). Using this site does not create a patient relationship. To become a patient, please contact the office.

1. Information this website collects

  • Information you send us. When you fill out the contact form we receive your name, email, phone number, and the message you write. We use it to reply to you and, if you choose to become a patient, to begin your chart.
  • Automatic technical information. Our web host records standard log data (IP address, browser type, pages visited, time of visit). We use it to keep the site secure and to understand which pages are useful.
  • Cookies. A single cookie from Google reCAPTCHA is used to keep the contact form free of spam. Anonymous analytics cookies only load if you accept them on the banner shown at your first visit. You can change your choice at any time from the small cookie icon in the corner of the page.

2. Information we do not collect on this site

We do not ask for Social Security numbers, insurance card numbers, dates of birth, or clinical details through this website. Please do not send those over the contact form or by email. Once you are a patient, protected health information is exchanged through the secure patient portal or in person.

3. How protected health information (PHI) is handled

When you become a patient, HIPAA governs how your PHI is used and disclosed. In brief:

  • We use PHI to provide your care, to bill for services, and to run the practice.
  • We disclose PHI to other providers involved in your care, to your health plan when needed for coverage, and to public health authorities when the law requires it.
  • We do not sell your PHI, and we do not use it for marketing without your written authorization.
  • You have the right to see and get a copy of your record, to ask for corrections, to request a list of certain disclosures, to ask us to communicate with you in a particular way, and to file a complaint if you believe your privacy has been violated. Complaints can be made to our office and to the U.S. Department of Health and Human Services, and there will be no retaliation.

4. Security

We use industry-standard measures to protect information sent through this site, including HTTPS on every page. No method of transmission over the internet is perfectly secure. Please do not include sensitive medical information in a website contact message.

5. Children

This website is not directed to children under 13, and we do not knowingly collect information from them online.

6. Changes

We may update this notice. The current version is posted on this page with the effective date shown at the top.
